This report summarizes the findings of the Roundtable convened by Center for Open Data Enterprise (CODE) and the Department of Health and Human Services (HHS). It provides an overview of high-priority social determinants of health (SDOH) data, a look into SDOH is and can be used along the care continuum, a description of high-priority opportunities, and recommended actions for HHS.
