There are many initiatives that address social determinants of health (SDOH) within healthcare settings; however, many are happening in silos. There is limited Medicaid-specific guidance for collecting SDOH data and addressing social needs in healthcare organizations.

This report is on the results of an Expert Panel that was convened to identify a framework for state Medicaid programs to support the collection of SDOH data and the integration of health and non-health services, using food insecurity and housing instability as illustrative examples.
