This All In technical assistance webinar on the social determinants of health (SDOH) presented two case studies. The first presentation is on PRAPARE, an EHR-integrated questionnaire on the SDOH administered in clinical settings. The second presentation is on the Duke-Durham Collaborative Community Health Indicators Project, which involves making geotagged, aggregated SDOH data available to the public to drive community action.
